Every participants will be asked to declare any conflicts of interest for each of the voted topics.

The aim of this International Consensus Conference is to try and identify all the drugs/techniques/strategies that might reduce or increase all-cause mortality in critically ill patients with or at risk for acute kidney injury (AKI) and then to suggest to researchers and donors on which topics they should be focusing when performing trials or financing research in the immediate future.

The Conference will be considering, with a top/down priority randomized evidence either in the form of randomized controlled trials (RCT) or meta-analysis of randomized controlled trials published in peer-reviewed journals (the subgroup analyses of RCTs or the subgroup analyses of meta-analysis of RCTs will be also be considered).

If mortality is included in a composite endpoint the drug/technique/strategy can be included according to the weight of mortality in the composite outcome and to the scientific consensus.

Non-randomized evidence or trends towards a statistically significance will be considered as time permits. If the identified publication for a specific drug/technique/strategy included multiple settings (predominantly, but not exclusively, critically ill patients) the participants will decide by consensus whether to include it or not in the final conference proceedings.

December 2011 – February 14th 2012: Scientists, Clinicians, Scientific Societies, Editorial Boards of International Journals, Nursing and Patients’ Societies worldwide are asked to vote on the already suggested topics (and to add new ones) as well as to produce the scientific evidence to support the different drugs/techniques. The web page http://www.hsrproceedings.org/consensus/index.php?pag=topicselection will be used.

The following sophisticated PubMed search strategy has been used by different, blinded colleagues to systematically identify related topics that have been put on the web page:

((acute AND (renal OR kidney) AND (failure OR injury)) OR (renal AND replacement AND therapy)) AND ((death* OR survival OR mortality)) AND (prevent* OR reducti* OR reduci*) AND (significat* OR significan*) AND (randomized controlled trial[pt] OR controlled clinical trial[pt] OR randomized controlled trials[mh] OR random allocation[mh] OR double-blind method[mh] OR single-blind method[mh] OR clinical trial[pt] OR clinical trials[mh] OR (clinical trial[tw] OR ((singl*[tw] OR doubl*[tw] OR trebl*[tw] OR tripl*[tw]) AND (mask*[tw] OR blind[tw])) OR (latin square[tw]) OR placebos[mh] OR placebo*[tw] OR random*[tw] OR research design[mh:noexp] OR comparative study[tw] OR follow-up studies[mh] OR prospective studies[mh] OR cross-over studies[mh] OR control*[tw] OR prospectiv*[tw] OR volunteer*[tw]) NOT (animal[mh] NOT human[mh]))

February 14th 2012 at the San Raffaele Hospital, Milan, Italy. (DIBIT 2, C2, 2nd Floor).

From 8.00 am to 20.00 pm
Each topic will be briefly presented and the participants will discuss and vote on them.
Aim of the Consensus Conference is to establish whether:

  • The evidence we collected so far is the most updated

  • The reduction or increase in mortality is supported by either: randomized controlled trials or the meta-analysis of randomized controlled trials, or case-matched studies, or meta-analysis of case matched studies, or other studies

  • The evidence derives from a subgroup analysis or a main analysis

  • The evidence derives entirely or partially from a critically ill population

  • The setting in which the drug/technique/strategy is used

  • Mortality is the endpoint, or it is included in a composite endpoint.

For each topic the Consensus Conference will produce and vote a position statement and will choose a main manuscript/the main manuscripts.
